Patient Information - Getting a Referral from Your Doctor
 It’s easy to get a referral from most doctors, just contact our office at 972-673-0110. Next, give us your doctor’s name and fax number. We’ll email you a client release form which allows us to speak with your physician or psychiatrist. Once you sign the release form and fax it back to us we’ll take that form and fax it, along with a referral form to your doctor. Most doctor’s get back to us within 24-48 hours with a signed referral, so that you can begin hypnosis sessions with us right away.
